WMATA Visitor Questionnaire

The health and safety of WMATA's employees, customers, vendors, and visitors remain our priorities. WMATA continues to monitor the evolution of the coronavirus disease 2019 (COVID-19) and will update its guidance based on recommendations provided by the U.S. Center for Disease Control and Prevention (CDC), U.S. Occupational Safety and Health Administration, and local health agencies, among others. At this time, only essential business visitors, including applicants testing/interviewing for select positions and new hires reporting for processing, are permitted at any WMATA facility.

To prevent the spread of COVID-19 and to reduce the potential risk of exposure, before entering any WMATA facility, all visitors must complete a screening questionnaire. Please answer the following questions no more than 2 days before your appointment and attest to the truthfulness of the responses:

If your answer to any of these questions is yes, you may not enter any WMATA facility.

WMATA will retain this document for three years from the execution date. The information will be kept confidential and used to determine your access to any WMATA facility. We appreciate your understanding and cooperation.
